Safety risks
Contraindications
Hypersensitivity to acamprosate or its components
Warnings and precautions
Alcohol withdrawal syndrome
Use caution in patients with risk factors for severe alcohol withdrawal, as acamprosate does not eliminate or reduce withdrawal symptoms.
Somnolence
Use extreme caution in patients performing activities requiring mental alertness, such as driving or operating machinery.
Suicidal ideation, depression
Use caution in all patients.
Specific populations
Renal impairment
CrCl ≥ 50 mL/min
Use acceptable. No dose adjustment required.
CrCl 30-50 mL/min
Start at dose of 333 mg. Do not exceed frequency of TID.
CrCl ≤ 30 mL/min
Do not use.
Renal replacement therapy
Any modality
Do not use.
Hepatic impairment
Any severity
Use acceptable. No dose adjustment required.

Use only if benefits outweigh potential risks. Evidence of fetal harm in animals. Weigh the risk of administering acamprosate compared to the risk of continued alcohol consumption.

Use with caution during breastfeeding.
Unknown amount excreted in breastmilk.
Unknown drug levels in breastfed infants.
Adverse reactions
Very common > 10%
Diarrhea
Common 1-10%
Hypertension, anxiety, asthenia, depression, dizziness, flatulence, pain, loss of appetite, insomnia, nausea, itching, sweating, paresthesia, dry mouth, rhinitis, peripheral edema, suicidal ideation
Unknown frequency
Deep vein thrombosis, heart failure, cardiomyopathy, irregular heartbeat, syncope, hearing loss, chest pain, insomnia, cough, pharyngitis, shock, dyspnea, visual disturbances
